Bilingual Case Manager (LPN)
Qualifications:
- Bilingual Spanish/English, required
- Licensed Practical Nurse
- 3+ years of customer service experience, preferably in human services, possess demonstrated commitment to customer service (Preferred)
- Professional or personal experience supporting individuals with disabilities, mental illnesses, or challenging behaviors is highly preferred
- Must have a valid in-state driver’s license
- Must be over 21 years old
- Proven working experience in case management, including as a nurse, medical, mental health care manager, or a related job
- Excellent knowledge of case management principles, healthcare management, and reimbursement
- Familiarity with professional and technical emerging knowledge
Responsibilities:
- Accomplishes clients’ care by assessing treatment needs; developing, monitoring, and evaluating treatment plans and progress; facilitating interdisciplinary approaches; monitoring staff performance.
- Determines clients’ requirements by completing intake interviews; determining the need for therapeutic medical, psycho-social, and psychiatric evaluations; reviewing therapist evaluations, treatment objectives, and plans.
- Establishes treatment programs by setting schedules and routines; coordinating services being provided; arranging resources, including transportation and escort.
- Monitors cases by verifying clients’ attendance; observing and evaluating treatments and responses; advocating for needed services and entitlements; obtaining additional resources; intervening in crises; providing personal support.
- Maintains clients’ records by reviewing case notes; logging events and progress.
- Communicates clients’ progress by conducting weekly interdisciplinary meetings and evaluations; disseminating results and obstacles to the therapeutic team and family; identifying treatment influences.
- Prepares reports by collecting, analyzing, and summarizing treatment and results from data and trends; compiling statistics; completing grant and subsidy applications.

Registered Nurse (RN)
Nurses provide a higher level of client care including:
- Supervise LPN nursing care
- Assess clients and create nursing care plans
- Evaluate and record clients’ conditions at each visit
- Recommend other methods of care that would help improve quality of life
- Coordinate care with doctors, physical or occupational therapists, and other healthcare entities

LPNs work under the supervision of RN supervisors to perform basic nursing care:
- Take and record vital signs
- Assist clients with bathing and hygiene care
- Medication reminders
- Help clients with ambulation
- Monitor blood sugar levels

Personal Care Attendant (PCA)
These assistants help clients with daily activities in the home including:
- Bathe and dress clients
- Serve meals and help clients to eat
- Turn or reposition clients who are bedridden
- Provide and empty bedpans
- Lift clients into beds and wheelchairs
- Clean and sanitize clients' areas
- Light housekeeping
- Maintain an established plan of care
